The following is a list of information you should bring in to the Clinic:

1. Proof of current address with documents that validate address: (telephone bill, utility bill, gas bill)
Prueba de su domicilio mas receinte or cualquier otro document que verifique su domicilio: (el cobro de luz, agua, gas)
2. Current working telephone number where you may be reached (home, work, cell, emergency, neighbor, or relative with whom we may leave a message)
Numero de telefono actual al cual nos podemos comunicar con usted. (de su hogar, trabajo, cellular, emergencia, vecino of familiar con quien le podemos dejar un mensaje).
3. If you have medical insurance: Proof of medical insurance (for example: Medicare, Medicaid, CHIP, or private insurance)
Si usted tiene seguro medico: Prueba de seguro medico (por ejemplo: Medicare, Medicaid, CHIP or seguro medico)
4. Names, dates of birth, and social security cards of all immediate family in the same household. Any child who is 18 years (and over) and is not in high school or college, needs to register on their own.
Los nombres, fechas de nacimiento, y tarjetas de numero social de todos los miembros de la amilia que viven en su hogar. Si el nino/nina tiene18 anos (o mas) y no esta atendiendo la escuela o colefio, el/ella tendria que registrarse por ellos mismos.
5. Proof of income of all immediate household members. Bring in current check stubs (last 4 if paid weekly, last 2 if paid bi-weekly, or last 1 if paid monthly), latest income tax copy with W-2 (such as 1040). You will also need to provide copies of award letters that show monthly benefits for social security, child support, VA, pension, retirement, TANF, alimony, and food stamps. (In the event none of the above documents are available, a Su Clinica supervisor will be called to assist.
Prueba de entrada de dinero de cada miembro de familiar en su hogar. Necesita traer los talons de cheque mas recientes (los ultimos 4 si le pagan por semana, los ultimos 2 si le pagan por quincena, or el ultimo 1 si le pagan por mes), la ultima forma de ingresos (income tax) con los W2 (la forma 1040). Tambien necesita traer las cartas de beneficios menusalmente de numero social, mantenimiento para ninos, vteranos, pension, retiro, TANF, mantenimiento para esposa (o), y estampillas de comida. En caso de qe no tenga los documentos antes mencionados se tendra que llama a su supervisor.
6. Proof of migrant, if you have migrated and participated in any agricultural labor work in the U.S. (including Texas) within the past 24 months, please bring last check stub and income tax return with W-2 in addition to all the above information.
Prueba de migrante, si ha migrado para participar en trabajos loborales, en cualquier estado de los Estados Unidos (incluyendo Texas) durante los ultimos 24 meses, favor de traer el ultimo talon de cheque y el income tax con los W-2 y toda la informacion requerida (de 1-5).
7. If you are a full time college student, please provide your financial aid invoice or class schedule, social security card, proof of address and college ID.
Si eres un estudiante de Universidad favor de traer su ayudante financier o su orario de la clase, su numero social, prueba de domicilio, y identificacion de la Universidad.
